Staff Forms: Screening
Florida law requires all of our staff to be
screened through the Department of Children and Families.
New: Everyone
must complete the affidavit every year.
It is very important that you send the following two
forms back promptly. This information is needed to complete a third form
that is sent to the Florida Department of Law Enforcement for processing.
Local Law Enforcement Check

Fill out the local law completely and sign it.

Take it to your local police or sheriff’s
department.

Take along a stamped envelope, pre-addressed to the
executive director. The address is on the form.

Many departments will complete the check while you
wait and return the form to you. If so, send the completed form to
the executive director.

Some will need the stamped pre-addressed envelope.
In that case the department will mail the form.
If you live in
Hillsborough County, you only need to return
the completed form. The check will be done on-line.
Affidavit of Good Moral
Character

Must be completed every year by everyone.

Fill out the affidavit . Be sure to write your
name on the front side.

Sign the one section on the back that applies to
you. If you have no record of the listed offenses, sign the top box.
If you do, sign the second box. The third and fourth boxes are for
teachers.

Have the affidavit notarized and send the notarized
form to the executive director.
